Well not right this second but if it makes you feel better, in my adult life I have fluctuated between 180 and 270. I’m sitting around 215 right now and was getting back on the exercise and healthyish eating kick when a death in the family fucked that all up. So I guess I’ll be here for a bit. Hopefully that means I’m still eligible to grab a drink with you old people.

There’s actually a whole lot of nothing in that BMJ article.  But noteworthy - first longitudinal study of trans youth ever in the US started in 2015. That’s the definition of politics. Infuriates me to no end. Shoulda happened decades before. Second, European societies have been some of the staunchest gatekeepers in the world on GAC, they haven’t changed.  Third, anyone who advocates for or gives a child medicine before at least 12 months of psychotherapy is doing it wrong. Fourth, even as an adult there is a bias against cross sex hormones, just last month I had to have a second blood test because “estrogen is dangerous” but a complete dismissal of the danger of doing without it (I was out of meds for 2 weeks, fuck it was like an itchy sweater). Fifth, even as an adult it took two letters - one from a mental health professional and one from my GP to start the lowest and quite frankly easily reversible level of estrogen. It took me 18 months to get those letters. Sixth, that article makes the claim there is little evidence hormone treatment prevents suicide, that’s laughable. Seventh, the Brits are the leaders of the gatekeepers in Europe as well, British trans women complain and complain about how hard it is to get treatment. So while we all want more evidence, that article reads no different than the last 20 years of articles.  The only difference is that many in the medial community now really want to help. That’s good. And let’s have solid clinical practice, there should not be a rush and frankly in most cases I don’t think there is. Yeah, I think this fluff piece is bullshit, because it reads like the articles I’ve read coming out of Europe for the last 20 years.

Starting with the stated percentage of detransition, which was not qualified by the percentage of those who detransition due to external factors, which is north of 80%. These external factors, such as bigotry and even disapproval, are listed in every study I have read, and whether and how this societal downpressing is factored into the conclusions is important to a clear understanding of the success of medical intervention.

@Anastasis says he could design a study which would yield a conclusion, pro or con, free from the flaws of previous studies, and also free of a control group, which is a much more humane position than the JBM took. He could make an important contribution to trans health outcomes, and I hope he takes the time to do so. It would be interesting to learn how he would factor societal pushback into his conclusions.

I could help by being part of the effort to find folks to conduct the study

The failure to see the rich and long standing bias against gender dysphoria, aka gender identity disorder aka transsexualism  and how unfuckingbelievably hard it has been to get treatment over the years is the worst part. The bias against treating thousands of patients properly because of fear that one cisgender patient will be confused is appalling. Look mother fuckers, the default when talking about care isn’t we are confused, the overwhelming number of patients that show up ARE trans, TREAT US!! And yes let’s work to avoid the mix ups but god dammit your precious gender construct is the most WHACK thing I’ve ever been a part of. There’s an entire world that lives breathes and knows transgender, fluidity and non-binary and it’s normal, it’s good, it’s fucking beautiful and instead of blocking us because you know better or because some may chose treatment when it’s a mistake is not our problem.  Hell, in ancient societies were were damned shaman and wise men and women. People listened when we spoke. Now we are told, not today, maybe next year you can have your precious blue pill (yes it’s a blue pill too).

and as for detransition  - I have to imagine I’m considered a success story here and I’ve considered it BECAUSE of external pressure. But you know what? Every morning and every night I take my meds and it’s the most certain thing I’ve ever done. It’s when I have the most clarity about who I am. But y’all wring your hands and tell me oh no, the confused cisgender kids!!

I can smell stink miles away.  There’s a long and consistent history of denialism in Britain and the rest of Europe. This fits within the historical norm. Yet clinicians keep marching on and treating patients and more and more find inner peace and they keep “oh no-ing” all the way home. Fuck that shit. It’s gross. There’s plenty of science to support current treatment.

The treatment protocol is called the Dutch protocol, right? Surprised that the Euros get thrown under here. I don't think that there is much room for disagreement with the fact that Sweden is among the most progressive European states when it comes to Trans rights. I think that you would agree with that, right? I am not sure that you can dismiss their position wrt the evidence grade out of hand. But maybe I am biased. I have worked with epidemiologists and researchers from Karolinksa and find them to be extremely thorough, level-headed, evidence-based, and just generally Nordic. Which are all positive attributes in this context, imo.

Since these studies look at meaningful mental improvement as a desired outcome, what have you to offer by way of distinguishing between internal and external causes leading to a failure to achieve the desired outcome?

I think that there are legitimate questions to be discussed regarding the content of the informed consent (for parents) and assent (for children) in this context.  Maybe the point goes over your head, though.

Yeah you're doing your cutesy trick where you act like you're being reasonable while arguing in support of the side that is trying to make folks like Troph be illegal to exist, or at the very least, too expensive to exist. 

But then again, pricing people out of care that they need IS how you put a roof over your head, so at least you're consistent.

the unsatisfied want diagnostic tests like it’s measurable from a blood test. I can’t think of a single mental health diagnosis that isn’t subjective with DSM-V criteria and diagnostic tools as sophisticated as a self report survey. But self reporting on gender dysphoria, get the fuck out of here with that shit. We need more robust science. The hypocrisy is never ending.

The title of that article itself is a huge red flag. Gender dysphoria isn’t “rising,” more people are willing to seek treatment because society is (marginally) less hostile towards it than previous decades. Of course, the hostilities are returning with a vengeance and amplified by social media and shit people winning elections. It’s truly despicable that even on this board people are playing along with shit articles like that as supporting their hate.

In Ana's ever-so-slight defense, I believe the lunatics that he's referring to are the ones discussing casually executing someone or bashing her head in with a hammer.

Now, I wouldn't personally kill Raichik--nor am I hoping that someone else does--BUT, if she were to get hit by a bus tomorrow, I'd probably start chortling uncontrollably.

I'm personally done pretending to give a shit about someone's life if they have an audience that they openly encourage to target marginalized people. Especially when said person purposefully ratchets up their rhetoric after incidents like the Club Q shooting. If it's not obvious to you (meaning people like Ana) at this point what her goals are, and what the goals of people like Tucker Carlson are when they go out of their way to give her a megaphone, then you need to dunk your head in some ice water or something. Anything that will wake you up from your lib-brained fog of war.

That said, it's not like I would ever really do something like that. Hell, I don't want the state to be able to execute people, much less endorse vigilantism. Sometimes a little hyperbole can be cathartic.

Can I renew my passport a year early while Biden is still in office? Is Ken Paxton going to ask for DL records that go back more than 2 years. What am I going to do if he does? will the Texas republicans be successful in an effective ban on GAC?  the bill has been filed. Vacation in Colorado this summer, what towns will we visit to look for an escape plan? Do we need to try and buy or rent this year to get a head of it? How can I get my meds cross border if I have to because GAC is effectively banned in Texas?  Should I detransition? God no. Do I run away or stay and fight? Do I speak up or try and go underground as much as possible? Is that even doable? Not really, at least 1000 people actually know me and that I’m trans because 8 years ago I told them, because I had to. 
 

but you keep dismissing it as bathroom bills - even while some of the bathroom bills would make me a criminal - scratch that - A SEX OFFENDER - just for peeing in a bathroom stall. 
 

this whole thing is fucking backward. We are not here to protect the strong, we are here to protect the marginalized.  Blocking trans care including care of trans kids to protect the rare cisgender kid or adult who feels later they made a mistake is the wrong framework.

I think that you have a stronger and more compelling position in the US political dialogue when your arguments are built around the real world impacts of legislation, as you just articulated, instead of just blanket defending controversial hormonal drug therapy in young adolescents and rolling that into the discussion. But I mean I guess go for the “if you think we shouldn’t give the kids sex hormones during puberty without understanding the medical and psychological outcomes, you are a nazi” angle if you think that will work better.

God you are terrible. I’ve never blanket supported GAC, I oppose blanket prohibitions. I actually support rationale access to GAC based on what families, their doctors and their mental health providers recommend. Jesus Christ.
