If you are going to the doctor for your yearly physical and something else briefly comes up, that's all in the game. If you go to the doctor for your yearly physical and you spend an extensive amount of time asking about other health issues that you would otherwise need to schedule separate appointments for and potentially with specialists, then it's probably fair for the insurance company to consider it more than just an annual physical. 

It sucks, and insurance companies are leaches on our general wellbeing, but it is within the rules of the established system. 

 

We need to change the rules of the system instead of complaining that the rules we think should exist aren't being followed.
